This page shows standard charges for shoppable services—tests and procedures you can schedule in advance. For each service we list a plain-language description and code (CPT/HCPCS/DRG), the discounted cash price, de-identified minimum and maximum negotiated charges across payers, and payer-specific negotiated charges labeled by payer and plan. Prices are shown by location and setting (inpatient, outpatient, or both).

What these terms mean
- Discounted cash price: The charge for an individual who pays cash or cash-equivalent. If no discounted cash price is offered, we show the gross charge.
- Payer-specific negotiated charge: The negotiated rate for a specific payer and plan (for example, Blue Cross PPO).
- De-identified minimum / maximum: The lowest and highest negotiated rates across all payers.
Important notes
Actual amounts depend on your benefits (deductible, coinsurance, and out-of-pocket maximum) and the services you receive. Some services include ancillary services (e.g., anesthesia or pathology) that may be billed separately; we list related services with the primary shoppable service where applicable.
